In order for us to obtain or service the insurance products that you ask for from us, we may collect personal information from you, as well as from others. We do not collect any personal information about you at our Website unless you choose to provide it to us voluntarily or unless you authorize us to obtain it from others. If you encounter a screen or page that requests information you do not want to share with us, do not enter the information and do not proceed with that screen or page.

Personal Information includes any identifying information, such as:

  • Your name, address, e-mail address, phone number, fax number (Note: any information provided for communication is retained in order to process your inquiries, respond to your requests, and improve our services);
  • Age, marital status, occupation, social security number, asset information, income, and beneficiaries;
  • Claim history, driving record, vehicle make and model, license number, VIN, lender(s) and leasing agent;
  • Credit card, debit card, checking account, credit history, credit standing, policy coverage, premiums, and payment history*.

*No usable credit card or debit card information is stored or retained by Family Select Insurance. Copies of checks made out for payment to Family Select Insurance may be stored in secure customer files.

We collect your personal information from:

  • You voluntarily in person or over the telephone or via the Internet, your authorized agent or representative, the mail courier services, our Website, or people you may live with or family members;
  • Documentation you complete and submit to us such as  applications, forms, claims and policy updates
  • Your business dealings with us and with other companies;
  • Consumer reporting agencies and Credit Bureaus;
  • The Department of Motor Vehicles or County Clerk to verify driving record, auto title, license status or other property title;
  • Databases like ChoicePoint, LexisNexis, and Comprehensive Loss Underwriting Exchange (CLUE)

How We Use and Disclose Personal Information

In order for us to obtain or to service the insurance products that you ask for from us, we may disclose your personal information to affiliated and non-affiliated companies. If you do provide us with personal information, we will only use it for the purposes described here, and we will not sell, license, transmit or disclose this information outside of the Company unless (1) you expressly authorize us to do so, (2) it is necessary to allow our contractors or agents to provide services for us, (3) in order to provide our products or services to you, (4) it is disclosed to entities that perform marketing services on our behalf or to other entities with whom we have joint marketing agreements, or (5) otherwise as we are required or permitted by law. We may also use the information you choose to provide to us to improve the content of our Website or to contact you for marketing purposes. In those instances when prior authorization by you is not required, we reserve the right to disclose personal information without your prior consent for purposes referenced herein.

  • Financial companies such as property & casualty and life & health insurers, premium finance companies, insurance Managing General Agents and General Agents, Surplus Lines Brokers and insurance agents or brokers;  
  • Service providers that help process or service your account;
  • Medical-care facilities or providers, to verify coverage and benefits;
  • A court, governmental or regulatory agency when there is a lawful request;
  • Law enforcement to protect against fraud or unauthorized transactions;
  • A group policyholder for purposes of reporting claims experience;

We may share personal information with insurance data banks that collect information about claim history, accident fault, and amounts paid. Insurance data banks may retain personal information and disclose it to other insurance companies and others legally entitled to see it.

We will also send the statement to anyone you ask us to who received your information from us in the past two years.
